New Patient Registration
To register with us as a new patient, please call in to the practice and we will provide you with the necessary registration forms.
When you register at the surgery we will need information that will help us trace your medical records from your previous doctor. We will also need your address post code and telephone number.
Because your medical records may take a while to arrive from your previous doctor we will ask you to complete a ‘New Patient Questionnaire’ which will provide us with some basic information about your health. You may also need to have a new patient check with one of our Practice Nurses so that we are aware immediately of your medical history and current medication. The receptionist will arrange this for you when you register or we will contact you if we would like you to have an initial consultation based on the information you have provided in your questionnaire.
We can only register patients who live within our Practice boundary. Our boundary stretches to Dove Holes to the North, Taddington to the East, Longnor to the South. (Patients who are visiting the area may be seen on a temporary basis.)
Every patient registered at The Stewart Medical Centre has a named GP who is responsible for the patients' overall care at the practice. If you wish to know who your named GP is, please contact the surgery. If you have any preference as to who your named GP is, the practice will make every effort to accommodate this request.
If you are registered with our practice and you provide significant health or social care for a relative, please let us know so that we can ensure that you have access to the appropriate support for your role.
