Dementia Support Workers
We are very lucky to have two Dementia Support Workers who work along side us at the Practice. If you or a loved one has dementia, or is worried about developing memory problems, contact your GP who can refer you for an assessment.
Community Nurses
We have good links with Derbyshire Community Nursing Team who look after housebound patients' needs including blood tests, dressings changes and catheters.
Referral must be via the GP in the first instance.
Once you have been referred, you can contact the Community Access Point on 01332 564900.
Care Home Ward Rounds
We are aligned to four of the local Care Homes in Buxton and offer a weekly ward round for their patients meaning that we get to know the residents well and understand their needs when they are unwell. Our aligned care homes are:
- Argyle
- Branksome
- Haddon Hall
- Hollin Knowle
Patients in other care homes are welcome to register with us if they choose.
