Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

To pursue a complaint please complete this form and the practice manager will deal with your concerns appropriately. Alternatively, please put this in writing and mark for the attention of the Practice Manager.

The practice welcomes compliments and positive feedback. Compliments help recognise excellent service, support staff morale and identify examples of good practice that can be shared across the organisation.

You can share your compliments in a number of ways -  by phone (01298 22338), using the feedback text that is sent following all appointments or via the practice email address: admin.stewartmedical@nhs.net
